Liability Waiver & Release

I acknowledge that surfing and stand-up paddleboarding are inherently dangerous activities that involve risks of serious injury or death. I understand these risks include, but are not limited to: drowning, collision with other surfers or objects, marine life encounters, equipment failure, and injuries from waves or currents.

I voluntarily assume all risks associated with participation in surf lessons provided by Jacksonville Surf & Paddle. I agree to release, waive, discharge, and covenant not to sue Jacksonville Surf & Paddle, its owners, instructors, employees, and agents from any and all liability, claims, demands, or causes of action arising from my participation.

I certify that I am in good physical condition and have no medical conditions that would prevent safe participation. I agree to follow all safety instructions provided by my instructor.

For minors: I certify that I am the parent or legal guardian of the minor participant(s) listed above and have the authority to enter into this agreement on their behalf.
